You cannot see a failed or mis-sized liner from the room, and that is exactly the problem. A cracked tile is a gap where combustion gases reach masonry and framing. An oversized flue is subtler still — everything looks fine, the fire lights, and meanwhile exhaust is cooling on the way up and condensing on the liner walls. Neither announces itself until it does.
